Has anyone heard of a slave cylinder causing the clutch to slip and any pointers in replacing?

The only way I know that could happen is if the slave is jammed in the out or extended position. Or perhpas if there is a problem w/the master cyl is maintaining pressure not allowing the slave to retract.
Usually though when the slave goes bad it will not extend because it will not hold pressure.
My guess is that you may have a weak or broken pressure plate causing the problem.

Just my 0.02.
